The watch app is rarely the whole job, and that is what surprises people. It needs a phone app to pair with, and the sync that fills in the hours the watch was out of range. Notifications and a watch face are small work. Once sensor readings go into the phone's health store, HealthKit or Health Connect, the phone side costs more than the watch.
